Wise 360-Intensive Therapy Alliance

PLEASE READ CAREFULLY - SPECIALIZED PACKAGE WITH REQUIRED STIPULATIONS

The Wise 360-Intensive Therapy Alliance package is a treatment package for clients needing 2x 60-minute sessions weekly, 2x weekly 15 to 20-minute check-ins, 3 hours of case management per month (i.e., meetings with a psychiatrist, previous therapist, hospital social worker or treatment team, referrals to name a few), and 1x monthly house check-ins for a total of 3 months consecutively.

This package requires the following: outpatient, IOP, hospitalization, driving record within the past 1-2 years (if applicable), along with a release of information (ROI) signed to speak with the current medication management provider, previous therapist (within in the past 6 months to 1 year), and interdisciplinary team if the patient is hospitalized. Please note, that the client has the right to refuse to submit these documents which may result in the client being ineligible for this package - please discuss exceptions with your provider. 

Please note that travel costs are not included for locations outside of a 50-mile radius of New Orleans, Louisiana - this feature may be waived if the package is purchased in full and locations are within an 85-mile radius of New Orleans, LA. 

All information must be submitted and signed before the commencement of therapy. 

A comprehensive mental health assessment and evaluation w/ collateral session is highly recommended ($1200 value) to account for changes over time and to either rule out or rule in diagnoses. A 4-hour session utilizing self-reports, semi-structured interviews, and collateral information gathered from a spouse, parent, or family member who you reside with or who may serve as a supportive person (1-hour session). 

Required 1x Monthly Family Sessions: (if applicable) This requirement ensures that the family or support person is aware of treatment goals, is compliant with treatment goals, is supported, and is also held accountable for setting healthy boundaries with themselves for the client. Specifically, this feature ensures that enabling behaviors are reduced. This includes a parent, spouse, or contact with one support person who lives with the client (priority) or if the client lives alone, the nearest support person (secondary).

Week 1 The comprehensive mental health assessment and evaluation will be scheduled during week one, along with collateral contact meetings.  

Week 2 will start off with 3x 90-minute intake sessions.

Week 3 will begin the series of 2x 60-minute sessions and 2x weekly 15 to 20-minute check-in sessions for 10 weeks. 1x - monthly house calls will be scheduled on a monthly basis (weekends available).

The house check-ins are 3-hour sessions that include select tasks based on the client's needs, such as space organization, social exposure, a walk-and-talk session in a park or mall, etc. (3 house check-ins total)

Labs needed: (not included) Most recent physical examination or check-up with PCP; Micronutrient test, CBC, hormone levels

Materials required: (not included) Therapy Journal & pen (required); 1x expanding file folder (required); Therapy Calendar (required); Earbuds (optional); 1x Copper Water bottle (optional); Vitamin supplements (based on micronutrient labs; optional however highly recommended). The Wise 360-Intensive Therapy Alliance package is a 3-month package to ensure optimal client support and client-sustainable outcomes. Client readiness to change and commitment to change will be assessed to be eligible for this package. This package may be repurchased with the absence of the cost for the mental health assessment and evaluation.
